Expert Discussion: Hypofractionated Radiation Therapy – Standard for All Indications?
[摘要] Krug: Level I evidence from randomized controlledtrials and a Cochrane meta-analysis of these trials supportmoderate hypofractionation as the standard of care forwhole-breast radiotherapy. It is important to point outthat uncertainties regarding moderate hypofractionationfor specific subgroups, such as very young patients, patients with connective tissue disorders or patients withneoadjuvant chemotherapy, do not result from signs ofinferiority from the landmark trials but from the fact thatsome patient populations were not well represented. Wenow have additional data from newer trials, such as DBCG-HYPO [1], as well as population-based and institutional series [2–5], which overwhelmingly support moderate hypofractionation with similar or, in some endpoints, improved results regarding oncologic outcomeand toxicity as compared to standard fractionation. Thus,I believe that there are no clear contra-indications tomoderate hypofractionation.
