Although guidance suggestsbreast expression should begin within thefirst hour of birth, most first expressionsoccur much later, and a recent Australianstudy sought to understand why.1 Theyfound that, while individual philosophies onthe benefits of human milk were positive,expressing in the birth suite was dictated byessential clinical and administrative tasks,and by the value that institutions placed onexpressing in the first hour.Clear objectives to undertake expressingwithin the first hour or within the birthsuite stay, the authors suggest, need tobe included in policies and supported bymanagement and team leaders.
