As Always, the Devil Is in the Details

[摘要] In addition to meta-analyses by the Cochrane Collaboration, there is another meta-analysis on the treatment of asymptomatic bacterial vaginosis (BV) in pregnancy, which includes five randomized controlled trials and demonstrates a significant reduction in late miscarriages and preterm births by therapy with clindamycin (1). As always, the devil is in the details: The effect depends on the week of gestation in which the pregnant women were examined, whether in addition to a BV, also an intermediate vaginal flora (Nugent score 4–6) was treated, on the exact details of the therapy (substance, dose, route of administration), whether the success of the treatment was monitored, and whether the therapy was repeated if necessary (note that there is a high recurrence rate of BV), among other factors. Meta-analyses that are not in line with Cochrane reviews should not be ignored.
