Management of an acutely bleeding hepatic adenoma

[摘要] A 31-year-old woman with a history of Downsyndrome, hypothyroidism, type 2 diabetes, and a13-year history of extended-cycle contraceptive pilluse presented to the emergency department after asyncopal episode witnessed by her mother. She alsohad an episode of non-bloody, non-bilious emesisearlier in the evening.On clinical presentation, the patient was hypotensive at 90/60, tachycardic, and had a hemoglobinof 4.8 and a lactate of 11.66. Her abdomen wasperitonitic on examination. Immediate management included resuscitation with fluids and bloodto manage her hypotension.
