A paraneoplastic potassium and acid-base disturbance

[摘要] A 55-year-old man is admitted to the hospital with generalized malaise, paresthesias, and severe hypertension. He says he hadexperienced agitation along with weakness onexertion 24 hours before presentation to theemergency department, with subsequent onset of paresthesias in his lower extremities andperioral area.He is already known to have mild chronicobstructive pulmonary disease, with a ratio offorced expiratory volume in 1 second (FEV1)to forced vital capacity (FVC) of less than70% and an FEV1 85% of predicted. In addition, he was recently diagnosed with diabetes,resistant hypertension requiring maximumdoses of 3 agents (a calcium channel blocker,an angiotensin-converting enzyme inhibitor,and a loop diuretic), and hyperlipidemia.
