Mediastinal granuloma due to histoplasmosis in a patient on infliximab

[摘要] A 50-year-old manwith Crohn diseaseand psoriatic arthritis treated with infl iximab and methotrexate presented to a tertiarycare hospital with fever, cough, and chest discomfort. The symptoms had fi rst appeared 2weeks earlier, and he had gone to an urgentcare center, where he was prescribed a 5-daycourse of azithromycin and a corticosteroid,but this had not relieved his symptoms.
