When does acute pyelonephritis require imaging?

[摘要] A previously healthy 44-year-old womanpresents to the emergency departmentwith 1 day of fever, fl ank pain, dysuria, andpersistent nausea and vomiting. Her temperature is 38.7°C (101.7°F), heart rate 102 beatsper minute, and blood pressure 120/70 mmHg. She has costovertebral angle tenderness.Laboratory testing reveals mild leukocytosisand a normal serum creatinine level; urinalysisshows leukocytes, as well as leukocyte esteraseand nitrites. She has no personal or familyhistory of nephrolithiasis. Urine cultures areobtained, and she is started on intravenousantibiotics and intravenous hydration to treatpyelonephritis.
