Regarding “Surgical Metastasectomy in the Spine: A Review Article”
[摘要] We read with interest the article by Kato et al. [1] about“total en bloc spondylectomy” (TES). The authors arguedthat for certain patients with isolated and removable spinemetastases (SM), a complete SM removal via TES is a usefuloption as it can improve function and survival. They supposed that the completeness of the resection was associated with durable postoperative survival.As highlighted by the authors, over recent decades,modern systemic therapies have been developed, and as aresult, more patients with SM live longer. Moreover, themost relevant work of the literature was performed byWright et al. in the biggest cohort study of consecutivepatients undergoing surgery for SM [2]. They demonstrated that even if surgery for SM can improve pain,deformity, and neurologic function, patients with SM nowlive longer without modifications in surgical management.They concluded that this enhanced survival was due moreto advances in medical therapy than to extensivesurgeries.
