Cor triatriatrum or divided left atrium presenting as mitral stenosis in an adult patient
[摘要] A 26-year-old male patient presented to the hospital witha 2-month history of progressive dyspnoea. He deniedchest pain, coughing, orthopnoea, paroxysmal nocturnaldyspnoea, syncope or pre-syncope.He had no other significant comorbidities and hewas not on any chronic medication.His cardiovascular examination revealed anundisplaced apex beat with a parasternal heave and aloud second heart sound (P2) suggestive of pulmonaryhypertension. On auscultation, the first heart soundwas normal with a loud second heart sound and adiastolic rumble. Given these findings, a clinical diagnosisof severe mitral stenosis with pulmonary hypertensionwas made. ECG was atypical for mitral stenosis, it revealeda dilated LA with left axis deviation due to a left anteriorhemiblock. No features of right ventricular hypertrophywas noted.
