Myelomatous pleural effusion

[摘要] A 66-year-old woman was admitted to the emergency departmentreporting 1 week of asthenia and fever that had persisted despiteantibiotherapy. She had been diagnosed of smoldering multiplemyeloma (MM) IgG kappa 11 years before. However, treatment withbortezomib, lenalidomide and dexamethasone (VRd) was not starteduntil the previous year, when the patient presented with rib pain andthe diagnostic workup was repeated. Bone marrow aspirate showedinfiltration of clonal plasma cells, and bone disease was demonstratedin positron emission tomography/computed tomography scan. Afterthe fourth cycle of VRd, an increase in serum monoclonal componentwas observed.
