Qualitative evaluation of a cardiovascular quality improvement programmereveals sizable data inaccuracies in small primary care practices
[摘要] Among the most promising quality improvement (QI) interventions for small primarycare practices are those led by practice facilitators (PFs), specially trained individuals whohelp practices develop capacity for continuous QI.1 2 They provide coaching on bestpractices for QI implementation, includingusing technology to improve care.3 PF-led QIinitiatives are positively associated with guideline adoption,4 5 and may be cost-neutral ifthey reduce even a small number of high costevents (eg, admissions).6 As part of HealthyHearts in the Heartland (H3), a programmefrom the Agency for Healthcare Researchand Quality’s EvidenceNow initiative, PFsworked with small and medium-sized primarycare practices to implement QI strategies forcardiovascular disease prevention.7 To identify lessons learnt from the programme, weinterviewed practice leaders and PFs frompractices that experienced the largest andsmallest gains in quality scores to understandtheir experiences.
