Accuracy of Sonography in the Diagnosis of Urinary Tract Calculi Compared with Non-Contrast CT Scan
[摘要] BackgroundandObjective:Thisstudywasdesignedtodeterminesensitivity,specificity,PPV(positivepredictivevalue),NPV(negativepredictivevalue)andaccuracyofsonographyincomparisontonon-contrastCTscanfordiagnosisofurinarytractcalculi.SubjectsandMethods:Total131patientswithurinarycalculidetectedbynoncontrastCTscanunderwentsonographyonthesamedaywithoutbowelpreparationandinblindedfashionfromSep2011toMarch2012.CTscanandsonographyfindingsincludinglocationandsizeofstonesweregathered.Results:Thesensitivity,specificity,PPV,NPVandaccuracyofsonographyindetectionofcalicealcalculiwere91.7%,72.9%,80.5%,87.8%and83.2%,respectively.Thesensitivity,specificity,PPV,NPVandaccuracyofsonographyindetectionofrenalpelviscalculiwere85.7,100,100,97.3and97.7%,respectively.Thesensitivity,specificity,PPV,NPVandaccuracyofsonographyindetectionofuretercalculiwere53%,93.8%,89.7%,66.3%and73.3%,respectively.Thesensitivity,specificity,PPV,NPVandaccuracyofsonographyindetectionofbladdercalculiwere84.6%,100%,100%,98.3%and98.5%,respectively.Thesensitivity,specificity,PPV,NPVandaccuracyofsonographyincomparisontoCTscan,fordetectionofallurinarytractcalculiwere78.7%,91.7%,91.9%,87.4%and88.2%respectively.Conclusion:Sonographyisareliablemethodindetectionandconfirmationofrenalpelvisandbladderstonesbutisnotreliablefordetectionofureterandcalicealstones.Overallresultsshowthatsonographyisthesafeandfirstmethodfordetectionofrenalcalculi.HowevermultisliceCTscanremainssecondchoiceifsonographyresultsarenotsufficientlyclearinpatientswithclinicaldoubtofureterandcalicealoflt;3mmstones
