Minor medical error may cause to significant complications
[摘要] Controlandcorrectrecordationoffluidintakeandurinaryoutputisasimplebutanimportantmeasureinthemanagementandtreatmentofpatientswithacuterenalfailure(ARF)andnon-correctrecordationofurinaryoutputbynursemaycausetosignificantcomplicationsamongthesepatients.Thepatientwasa28-yaer-oldwomanwithseverepreeclampsiaat31weeksofgestationwhodevelopedARFandanuriaafterhysterectomyduringemergencycesareansection.ComputedtomographyandSerialultrasonographyofkidneysandurinarytractin2,4,8,and12dayaftercesareansectionrevealedonlymildhydronephrosis.Although,thepatienthasnotbeenanyurinaryoutput,butunfortunatelytherecodedurinaryoutputbynurseswas300-350ml/dayduetourinarybaghasnotbeenemptedintheendofeachday.WethoughtthatARFisduetoacutetubularnecrosisbecauseofseverebleedingduringsurgeryandhydronephrosisissecondarytorecentpregnancy.Afterfifteendays,itwasrevealedthatthepatienthadanuriaduringthisperiodandthereforeourdiagnosiswaschangedtobilateralureteralligationduringhysterectomyandrenalfunctionimprovedaftercorrectionofureteralobstruction
