Assessment of Thyroid Dysfunction and Thyroid Auto-Antibodies in Type 1 Diabetes
[摘要] BackgroundandObjective:Toinvestigatetheassociationbetweenthyroiddysfunction,thyroidautoimmunityandtype1diabetesmellitus,asampleof146type1diabeticpatientswerestudied.Subjectsandmethods:AllreferralstodiabetesclinicindiabetesresearchcenterwerescreenedforTSH/T3/T4/T3Ru/HbA1candAntiTGAb,AntiTpoAb.Foreachpatient,aquestionnaireincludingvariablessuchastimefromdiagnosisofDiabetesmellitus,age,gender,consumedhypoglycemicDrugsandtypeofdiabeteswereregisteredtodetermineassociationbetweenthyroiddysfunctionandeachoftheseitems.Results:Ofallthediabeticpatients37.7%wasknowntohavethyroiddysfunction.Themostcommonformswere:subclinicalhypothyroidism%13,hypothyroidism%8.9,hyperthyroidism%8.9,andsubclinicalhyperthyroidism%6.8.Therewasasignificantdifferencebetweenprevalenceofthyroiddysfunctionandgender(%40offemale,%30ofmale,P=0.045).Asignificantdifferencewasfoundbetweenageandthyroiddysfunctionintype1diabetesmellitus(P=0.001).TherewasnosignificantdifferencebetweenHbA1CandThyroiddysfunction.PositiveTPo-AbandTG-Abwerefoundin%60and42.9ofpatientsrespectively.Therewasasignificantdifferencebetweentimefromdiagnosisofdiabetesmellitus(Type1)andthyroiddysfunctions(Plt;0.05)Conclusion:Ourprospectivestudyconfirmstheassociationbetweenthyroiddysfunctionandtype1Diabetes,anddemonstratesthehighprevalenceofthyroidAutoAntibodyinpatientswithtype1Diabetesparticularlywomen.Weconcludethatallpatientswithtype1Diabetesmellitusshouldundergoannualscreeningwiththyroidfunctiontests.
