Functional Outcome of Unstable Pelvic Ring Fractures: A Comparative Study between Operative and Non-Operative Treatment

[摘要] BackgroundandObjective:Theaimofthisstudywastoevaluatethefunctionaloutcomeofunstablepelvicringfactures(TileBandC)andtocomparetheoutcomesofsurgicalandnon-surgicaltreatmentmethodsinthepatientsadmittedtoEmamKhomeiniandRazihospitalsinAhwaz,Iran.SubjectandMethods:Thisprospective–crosssectionalstudywascarriedoutduringfiveyearsperiodfrom2006-2010.Patientswithunstablepelvicringfractures(TileBandCtypes)treatedbyopenreductionandinternalfixation(ORIF)orclosedreductionandskeletaltraction.Themeanfollowuptimewas22monthsandtheresultsofbothgroupswereevaluatedbyMajeedscoringsystem.Result:Atotalof40patientswithunstablepelvicringfracture(20patientshadtypeBand20othershadtypeC(Tileclassification)wereincluded.ThemeanscoreintypeBfracturetreatednon-operativelywas78.36±9.01(good)andinsurgicalgroupwas83.55±7.53(good).IntypeC,meanscoreinnon-surgicalgroupwas64.11±8.3(fair)andinsurgicalgroupwas75.81±12.36(good).Meanscoreinallpatientstreatedsurgicallywas79.3±11.11andinnon-surgicallytreatedwas71.95±11.19thatshowsasignificantdifference(P=0.044).Conclusion:NosignificantdifferencewasobservedbetweensurgicalandskeletaltractiontreatmentfortypeBpelvicringfractures,butsurgicaltreatmentwasassociatedwitharateofmoregoodresultsthanskeletaltractionintypeCpelvicfractures.
