The Effect of Posture Training Support Orthosis on Static Balance in Elderly Patients with Spinal Hyperkyphosis
[摘要] BackgroundObjective:Fallsintheelderlyarecommonandareassociatedwithappreciablemorbidityandmortality.Hyperkyphosisisariskfactorforbalanceimpairmentandposturalinstability.Increasedforwardcurvatureofthespinedisplacesthecenterofmassanteriorlyoutofthelimitsofstability,andthusincreasesthelikelihoodoflossofbalanceandpredisposespersonstofall.Theorthosesareanoptionfordecreasingtheriskoffallinginelderlypopulation;however,thequantitativeevidencetosupporttheirapplicationisnotconvincing.Thepurposeofthisstudyistoanalyzetheeffectsofposturetrainingsupportorthosisonstaticbalanceofelderlypeoplewithspinalhyperkyphosis.SubjectsandMethods:Eighteenelderlypeoplewiththemeanageof65.96±4.4whohadthoracickyphosisof≥50(cobbangle)wereenrolledinthestudy.Subjectswererandomlyallocatedintoeitherposturetrainingsupport(PTS)orthosisorcontrolgroup.Patientswererandomlysubjectedtocomputerizeddynamicposturography,whichcontainedsensoryorganizationtestsandlimitsofstability.Thetestsforeachpersonwasconductedintwoconditions:whilewearingandwithoutwearingtheorthosis,andtheorderoftestingforeachpatientwasrandomized.Results:WearingPTSorthosissignificantlyincreasedbalancescore(plt;0.0001),directionalcontrol(p=0.023)andreactiontime(p=0.045).Inaddition,significantdifferenceinbalancescore(plt;0.0001)anddirectionalcontrol(p=0.041)wasobservedbetweentwogroups(PTSorthosisandcontrolgroup).Conclusion:ThePTSorthosisimprovesbalanceinelderlypeoplewithspinalhyperkyphosis.Improvedbalancecouldreducetheriskoffallsanddirectandindirectcostsoffallsandrelatedfractures.
