Effect of Spinal Low Dose Bupivacaine-Sufentanyl for Cesarean Section in Preeclamptic Parturients on Neonatal Outcome.
[摘要] Background::WestudiedmarkersofbothneonatalandmaternalhemodynamicconditioninpreeclampticpatientsreceivingspinalanesthesiaforcesareansectionandevaluatedprimaryneonataloutcomewithApgarscores.Methods::44preeclampticpatientsrandomizedintotwogroupsof22patientswereenrolledinthistrialinTabrizAlzahraHospital.ThetrialswereconductedfromDecember2005toAugust2006.IngroupA,spinalanesthesiawasperformedwith6mgofbupivacaineand3.3μgofsufentanyl,whileGroupBreceived12mgofbupivacainealone.Hypotensionwastreatedwithintravenousephedrinebolusesof2.5-5mg,uptomaximumof50mg.Afterdelivery,1stand5thminuteApgarscoreswereevaluatedandumbilicalarterialbloodgassamplesweredrawnandanalyzed.Results::Allpatientshadsatisfactoryanesthesia.Fiveof22patientsingroupArequired5mgofephedrine,while17ofthe22patientsingroupBrequiredephedrine.Therewasnosignificantbetweengroupdifferencesfor1stand5thminuteApgarscores(P=0.760,P=0.349)andumbilicalarterialbloodgasmarkers(p#62;0.05).Conclusion::A6mgdoseofbupivacaineincombinationwith3.3μgofsufentanylprovidedsatisfactoryspinalanesthesiaforcesareansectioninpreeclampticpatients,andcausedsubstantiallylesshypotensionthan12mgofbupivacainealone.Therewasnodifferenceinimmediateneonataloutcome,asassessedbyApgarscoresandneonatalpHandbasedeficit.
