Post-infectious Necrotizing Myelitis Associated with Meningitis: Report of a Fatal Case and Review of Literature.

[摘要] Abstract::Acutepost-infectiousnecrotizingmyelitis,arareneurologicalcomplicationofbacterialmeningitis,isclinicallydefinedbyacuteonsetofparaplegia,orquadriplegia,sensorylossandlossofsphinctercontrol.Theconditionshouldbedifferentiallydiagnosedfromvascularaccidentsandimmunogenicdemyelination.Thisisacaseofchronicotitismediacomplicatedwithmeningitiswhichwasfurthercomplicatedwithacutenecrotizingmyelitis.Aneighteen-year-oldmanpresentedwithfever,headacheandnauseaofonedayduration.Hewassufferingfromactivelypurulentrightsidedotitismediaformorethanayear.Patient’sdatawerefirstconsistentwithbacterialmeningitis.EmpiricalantibiotictherapywasstartedandcontinuedinspiteoftheCSFculturesshowingnogrowthduetopreviousadministrationofantibioticsforotitismedia.Againthepatientwascomplicatedwithacutehydrocephalusandsuddenattackofquadriplegiaandlossofsphinctercontrolafter48hours.CervicalMRIwasinfavorofacutenecrotizingmyelitis,regardingthepatient'sclinicalsituation.
