Comparison of the Hemodynamic Effects of Pretreatment with Crys-talloids versus Crystalloids plus Ephedrine during Spinal Anesthesia for Caesarean Section.

[摘要] Background::Preventionofhypotensionduringspinalanesthesiaforcesareansectionavoidsmaternalandfetalsideeffects.Theaimofthisstudywastocomparetheeffectsofthecom-binationofephedrinecrystalloidandprehydrationwithcrystalloidaloneonmaternalbloodpressureandneonataloutcomeduringcesareansectionunderspinalanesthesia.Method::Weenrolled72fulltermwomenweighingbetween50and75Kg,classifiedasASAI,scheduledforelectivecaesareansectionunderspinalanesthesia.Participantswereran-domlyallocatedtoeithertheephedrineorcrystalloidgroup.Afterarrivalintheoperatingroomandintravenous(IV)access,10ml/Kgofringersolutionwasinfused10-15minutesbeforetheinitiationofthespinalblock,butintheephedrinegroup,anadditional10mgofephedrinewasaddedtothesolutionforinfusion.Inthecrystalloidgroup,aplacebowasaddedtothesolution.Results::Hypotensionoccurredin55.6%ofpatientsinthecrystalloidgroupandin25%ofpatientsintheephedrinegroup.Thedifferencebetweenthetwogroupswasfoundtobestatisticallysignificant(P=0.008),however,therewasnosignificantdifferenceinmeansystolicbloodpressurebetweenthetwogroups(105.61±7.13incrystalloidgroupvs.107.89±9.84inephedrinegroup).Apgarscoresinnewbornswereabove8inbothgroups.Conclusion::Prophylacticephedrinegivenbyinfusionincombinationwithcrystalloidwasmoreeffectivethancrystalloidprehydrationinthepreventionofhypotensionduringspinalanesthesiaforelectivecaesareansection.
