Microabuminuria in Type 1 Diabetes Mellitus.
[摘要] Introduction:NephropathyisoneoftheriskycomplicationsofTypeIdiabetesmellitusthatcouldleadtoendstagerenaldisease.Persistentmicroalbuminuriai.e.smallamountsofurinaryalbuminexcretion(UAE),isthebestpredictorofhighriskofdevelopingdiabeticnephropathy.Earlydiagnosisofmicroabminuriaandevaluationofpatientswiththis,regardingthepresenceofcomorbidassociationsareeffectivestrategiesforreductionofdiabeticmortalityandmorbidity.Thepresentstudycarriedoutbecausethegreatvariabilityinprevalenceofnephropathydependsongeographicareaanddifferentpopulationandlittleisknownabouttheprevalenceofnephropathyanditsprecedingmorbidityinourcommunity.Aims:Todeterminethefrequencyofmicroabumnuriainchildrenandadolescentswithinsulindependentdiabetesmellitus.SubjectsandMethods:nacrosssectionalstudyfrequencyofmicroalbuminariawasinvestigatedin39childrenandadolescentswithIDDMwhichwereselectedbysimpleconvenientsampling.Datawereanalyzedbydescriptiveinferentialmethod(distribution,frequencyrate,mean,standarddeviation,ttestandx2test)usingSPSS.Pvalue<0.05wasconsideredsignificant.Results:fromatotal39patientswith5yearsdurationofdiabetes9(23.1%)patientsweremicroalbuminuricand30(76.9%)ofthemnormoalbumminuric.Themeandurationofdiabeteswas7.88±2.7yearinmicroalbuminuricpatientsandinnormoalbuminoricpatientswas7±2.5year,Theageofpatientswas16.06±2.3yearinmiroalbuminuricgroupandinnormoalbumiricpatientsitwas13.87±3.41year(p=0.01).Inmicroalbuminricpatientssystolicbloodpressureanddiastolicbloodpressurewere10.51±0.98mmHgand6.9±1.8mmHgrespectively.Innormoalbuminuricpatientssystolicbloodpressurewas10.26±1.31mmHganddiastolicpressure7.1±0.75mmHg.InpatientswithmicroalbuminuriathemeanofHbA1cvaluewas8.44±2.08andinnormoalbuminuricpatientsitwas8.62±1.88.Findingsalso,showednoassociationbetweenmiroalbuminuriawithtotalcholesterol,HDLLDLandtriglyceride.Discussion:Inourstudythemeanageofpatientsinmicroalbuminuricandnormoalbuminuricgroupswassignificantlydifferent,sothiscouldberelatedtoethnicdifferenceindifferentpopulations.Basedondataobtained,absenceofrelationshipbetweenmicroalbuminuriaandvariablesmaybeduetolownumbersofsamples,alsothefrequencyrateofmicroalbuminuriainourpatientswashigherthatmanyotherstudies.So,wecansaythisstudycanbeperceivedasapilotstudyandwesuggestdoingstudyonmorenumbersofpatientswhoareattendingindifferentclinicsofdiabetesandasanationalstudy.
