A Survey on Cricoid Pressure Application by Anesthetists.

[摘要] Background:For40yearscricoidpressurehasbeenusedtopreventregurgitationofgastriccontentsduringinductionofanaesthesia.Availablestudieshavehighlightedpoortechniquesregardingcricoidpressure.PatientsafetycouldbechallengedbyincorrectuseofcricoidpressureTheaimofthisstudywastoassessthepracticeandknowledgeofcricoidpressureamonganesthesiologistattendingintheninthinternationalcongressofanesthesiaandresuscitationinIran.MaterialsandMethods:Thiscrosssectionalobservationalstudyincluded59anesthesiologistsattendingthe9thinternationalIraniancongressofanesthesiaandresuscitationin2006.Eachsubjectwasaskedtoapplycricoidpressureonanundressedlaryngealmodelforoneminute.Themodelwasananatomicallycorrectrepresentationofthehumanlarynx.Itwasmountedontothebaseplateofakitchenscale.Participantswereblindedtotheresults.Fivemeasurementswereobtainedfromeachcandidate(0,15,30,45and60thsecond).ParticipantswasalsoaskedtocompleteaquestionnairecontainingdemographicdataandsomequestionsregardingattitudeandknowledgeofparticipantsregardingSlick'smaneuver.Result:Eighteenparticipants(30%)appliedthetargetcricoidforce(3-4kg).Whenconsideringbothcorrectanatomicalpositionandrightrangeofappliedforceonly11participants(19%)weresuccessful.Meanappliedforcesignificantlydecreasedattheendofoneminuteandmeanappliedforcebywomenwassignificantlylessthanmen.Therewasnostatisticallysignificantdiffenecebetweenacademicstaffandotheranesthesiologistsinapplicationoftargetforce.Onlyeightparticipants(13.6%)wereawareofthetargetcricoidforce.Conclusion:Applicationofcricoidpressurebytheparticipantanesthesiologistsispoor.UseofsimplemodelsfortrainingduringrefreshercoursesforpracticinganesthesiologistinadditiontostressingonthetheoreticalbasemayimprovethequalityofperformanceoftheSellick'smaneuver.
