Viable Brain with bilateral Internal Carotid Occlusion, A Case Report

[摘要] Abstract:Bilateralinternalcarotidartery(ICA)occlusionisextremelyrare.TheriskofstrokeinpatientswithtotalICAocclusionissignificant.OutcomeinpatientswithsymptomaticICAocclusionhasnotsubstantiallyimprovedovertheyears.Wereportacaseofbilateralcarotidocclusioninamiddleagedwoman.Shewasahealthywomanthatwaspresentedwithslightnumbnessinherrighthand.DuplexUltra-sonographydemonstratedahomogenousplaqueinoriginofRightInternalCarotidArtery(RICA)thatcaused90%stenosis,andtotalocclusionofLeftInternalCarotidArtery(LICA).CervicalMagneticResonanceAngiography(MRA)confirmedocclusionofLICAand90%stenosisofRICA.PatientwasreferredforrightsidedCarotidEndArterectomty(CEA).UnexpectedlyinfollowupDigitalSubtractionAngiography(DSA),oneweekaftersurgery;totalocclusionofbothICAswasobserved.After12monthsoffollowup;despitebilateralICAocclusion,sheiswell,active,andproductivewithcompletelynormalneurologicalexamination.
