The Clinical and Pathological Findings Among Patients with Lupus Nephritis in Shiraz, Southern Iran
[摘要] Systemiclupuserythematosusisachronic,debilitatingautoimmunedisorderthatinvolvesmultipleorgansystems.Nephritisisacomponentofsystemiclupuserythematosusthatinfluencethelong-termoutcomeofthepatientsandremainstheleadingcauseofdeathinthesespatients.Thisstudywasperformedtoevaluatethedemographic,clinical,andhistopathologicalfeaturesofIranianpatientswithlupusnephritisinShiraz,Iran.Aretrospectivesurveywasperformedon200patientswithrenalinvolvementandSLEwhoreferredtoRheumatologyResearchCenterofShirazUniversityofMedicalSciencessince1976.Among35patients(17.5%),renalbiopsywasnotperformedandexcludedfromthestudy.Patients’medicalrecordingfileswerereviewedfordemographic,clinicalpresentationofSLE,developmentofnephriticsyndrome,renalfailure,endstagerenaldisease(ESRD)duringnephritis,themortalityrateandprobablecausesofdeath.Themeanageofthepatientswas23.36±9.2yearsatthetimeofdiagnosisofSLE.Amongthe200performedbiopsies,35caseshadfailure.Theremaining165patientshadthefollowingWHOclassification:classIinone,classIIin30,classIIIin33,classIVin58,andclassVin43patients.ThemostfrequentclinicalpresentationsofSLEwerearthralgia,edema,andskinrash.ThemeanvalueofSLEactivityindex(SLEAI)was8.9.TheremaybedifferencesofclinicalandhistopathologicalmanifestationsofSLEandlupusnephritisconsideringgeographicaldistributionofthepatients.
