Kidney-related Diseases and Quality Improvement: AHRQ's Role

[摘要] Providing consistently high-quality care to people with chronic disease is one of the most serious challenges facing the U.S. healthcare system. Our system was not organized to provide care around patients' needs, so the net product for people with chronic illnesses too often is less than the sum of the contributions of individual clinicians who participate in a person's care. As a result, there are many opportunities for medical errors, other lapses in patient safety, or poor or missed care. This is true in the management and treatment of every chronic condition, and the care of hypertension, diabetes, and chronic kidney disease is no exception.
