Giant bilateral bullous emphysema due to deficit of alpha1-antritypsin

[摘要] The case of a female patient aged 48 with a giant bilateral bullous emphysema and low levels of alpha1-antitripsine determined at the Labcel laboratory of «Dr. Miguel Enríquez». Faculty of Medical Sciences was reported. The definition of alpha1-antitrypsin is the result of a genetical defect and approximately 75 % of the adults with a severe lack of this protein develop an emphysema that often begins before the 40 years of age. In order to discuss the results of the complementary tests, the treatment and evolution of this patient, we present this paper for considering it important for the medicosurgical practice.

[关键词] Bullous emphysema;alpha1-antitrypsin deficit;INTRODUCCIÓN El diagnóstico tardío de enfisema pulmonar por déficit de alfa1-antitripsina no es frecuente y en muchas ocasiones tratada como un cuadro de asma bronquial o una neuropatía inflamatoria.La exposición al humo del cigarro en fumadores;así como a gases tóxicos y elementos nocivos que contaminan el ambiente constituye factores de riego en la etiopatología de la enfermedad pulmonar obstructiva crónica;y favorece y amplía el daño a nivel del parénquima pulmonar en el déficit de alfa1-antitripsina. REPORTE DEL CASO Paciente de 48 años;del sexo femenino;cuyos antecedentes patológicos personales revelaron que era ex fumadora desde hace 7 años; fumó por un período de 12 años (5 a 6 cigarrillos por día). No se recogen antecedentes de casos similares en su familia. Historia de la enfermedad actual: Ingresa por falta de aire;con antecedentes de disnea a los mínimos esfuerzos (subir tres escalones;caminar media cuadra) desde hace 7 años;por lo que ha recibido tratamiento médico con aerosol de salbutamol y prednisona.
