Clarifying Requisite Sleep Medicine Content for the Pulmonary and Critical Care Medicine Fellow: A Key Step Forward, but Where Do We Go From Here?
[摘要] Despite strong historic roots in pulmonary and neurology, development of an American Board of Medical Specialties and Accreditation Council of Graduate Medical Education (ACGME)-recognized direct pathway into the field of sleep medicine has had some unintended consequences, including a reduction in trainees who elect to receive training in sleep medicine.1,2 Reasons include that spending an additional year of training is neither feasible nor appealing. Pulmonary medicine (PM) or pulmonary and critical care medicine (PCCM) fellowship training has also evolved such that sleep medicine exposure is quite limited to the extent that even concepts steeped in PCCM such as nuances of nocturnal noninvasive ventilation for sleep-disordered breathing are not uniformly integrated into the educational paradigm.
