A Man in His 50s Presenting With Rapid-Onset Dyspnea and Obstructive Shock

B, Circumferential pericardial effusion. A man in his 50s with no relevant medical history experienced rapidly progressive dyspnea over 2 days and presented to the ED. Upon arrival, he was found to be in moderate distress. Vital signs were: temperature, 36.6°C; BP, 94/58 mm Hg; heart rate, 33 beats/min; respiratory rate, 22 breaths/min; and oxygen saturation, 92% on 4 l oxygen per minute via nasal cannula. Physical examination was notable for pallor, diaphoresis, bradycardia, distant heart sounds, and retractions with respiration.
