Is It Time to Rethink Blood Glucose Targets in Critically Ill Patients?
[摘要] Enthusiasm waned for “tight” glycemic control in critically ill patients following publication of the multicenter Normoglycemia in Intensive Care Evaluation–Survival Using Glucose Algorithm Regulation (NICE-SUGAR) randomized controlled trial (RCT) of intensive insulin therapy in 2009.1 This study reported higher 90-day mortality in patients with a blood glucose (BG) target of 80 to 110 mg/dL compared with those treated with the “moderate” BG target of 140 to 180 mg/dL. Guideline writing groups responded by advocating the moderate target range,2 and clinical teams around the world tended to follow by loosening the reins, allowing higher mean glycemia in their patients.
