Beyond Smoking Cessation, Bronchodilator, and Oxygen Therapy in COPD: Hypoventilation Treatment and OSA Screening

[摘要] We enjoyed reading McNicholas’s¹ paper in a previous issue of CHEST (December 2017) critically discussing the epidemiological, clinical and therapeutic aspects of COPD-OSA Overlap Syndrome as well as COPD and OSA. The use of noninvasive ventilation (NIV) occurs in different scenarios with important positive results, as discussed in our work.^2,3 The McNicholas¹ study, however, did not completely approach the successful use of home NIV to treat hypercapnic, stable COPD. A recently published study recruited COPD outpatients with persistent Pa co ₂ >53 mm Hg and hypoxemia 14 to 28 days after hospital discharge; these patients had used NIV acutely because of hypercapnic COPD exacerbation during the most recent hospital stay.
