无缝隙护理管理模式在腹膜透析患者中的应用效果

[摘要] Abstract：Objective To investigate the effect of seamless management from hospital to home in nursing care of peritoneal dialysis (PD) patients. Methods A total of 50 PD patients were recruited as the control group between October 2013 and October 2014 and were given convention health education. Another 50 patient were included as the observation group and were given seamless management on PD. The compliance behavior to treatment and knowledge level of selfmanagement were observed and compared between two groups. Results Patients in the observation achieved higher compliance rate in regular health checking, body weight recoding and life routine compared with those in the control group (P＜0.05 or P＜0.01). In terms of selfmanagement assessment, patients in the observation performed better in diet control, exercise, sanitation, reexamination purpose, standard operation, drug treatment and complication prevention compared with those in the control group (P＜0.01). Conclusion Seamless management can improve the compliance behavior and knowledge level of selfmanagement in PD patients.
