优势模式在2型糖尿病患者社区个案管理的效果探讨

[摘要] Abstract：Objective To explore effect of Advantage pattern in patients with type 2 diabetes on community case management. Methods A total of 168 diabetic patients were divided into two groups, observation group implemented community case management and the control group applied regular pattern, glycosylated hemoglobin level and management level before and three months and six months after the intervention were contrast. Results After intervention, there was no significant difference in glycosylated hemoglobin in two groups(P>0.05). Compared with intervention before, the indicators of patients in the observation group in three months and six months after the intervention dropped 0.68% and 1.21%. Compared with the control group, and self management of the observation group was better(P<0.05). Conclusion The implementation of community case management can improve the metabolism and self management ability for type Ⅱ diabetes patients.
