不同时机控制胎头下降速度对会阴损伤的影响
[摘要] Abstract:Objective To investigate the influence of controlling of fetal head at different time on maternal perineal laceration. Methods A total of 200 primiparas undergoing vaginal delivery were assigned to the observation group and the control group, with 100 cases in each group. The controlling of fetal head at different stage of head visible on vulval gapping was carried out in the observation group(head visible 1 cm×1 cm)and in the control group(head visible 2 cm×3 cm). The time form head visible to crowning of head, time form crowning of head to fetal delivery, second stage of labor, perineal laceration degree and lateral episiotomy rate were compared between two groups. Results The time form head visible to crowning of head was longer and time form crowning of head to fetal delivery was shorter in the observation group than that of controls(P<0.05). There was no significant difference in time of second labor stage between two groups(P>0.05). The proportion of primiparas suffering grade Ⅱ perineal laceration was lower in the observation group than that of controls(20.0% vs, 42.0%, P<0.05 ). The lateral episiotomy rate was 10.0% in the observation group and was 27.0% in the control group(P<0.05). Conclusion Controlling of fetal head when head visible(1 cm×1 cm)on vulval gapping may reduce the degree of perineal laceration in primiparas undergoing vaginal delivery.
