[摘要] Purpose. Clarification of morphometric causality of formation of anterior chamber angle closure in patients with primary closedangle glaucoma (PCAG) in case of a thickened lens form and an assessment of a role of lens volume in this process.Material and methods. The paper presents a comparative analysis of three groups of patients: the main group PCAG presence with exclusively thickened lens form (61 eyes, 61 patients): the 1st subgroup – IOP compensation; the 2nd subgroup – IOP subcompensation; the 3rd subgroup – IOP decompensation; the comparative group – 15 eyes (15 individuals) with an acute PCAG attack; the control group – 30 eyes of 15 healthy patients. All were comparable for a sex, age, axial length. Calculation of the area of contours of lens cross sections, of anterior chamber was carried out, the position of a ciliary body (CB), extent of opening of the anterior chamber angle, the comparative analysis of indicators in groups were specified.Results. The reliable tendency was revealed between an increase in the average value of cross sections area of lens, a formation of full closure of the anterior chamber angle, a development of IOP decompensation in the main group. Lack of an acute attack was explained only by a medial position of the CB, as distinct from forward position of CB in acute attack of PCAG in the comparative group. The nuclear type of cataract was characteristic for eyes of the main group, in 18% of eyes there was no cataract and visual functions were high (0.6-0.8).Conclusions. The revealed regularities testify to the phacomorphic nature of PCAG formation in case of a thickened lens form. .
